 Overview (200-375 words):
This is where you connect the relevant information from the LAF form and the NSP scores in relation to the client’s health concerns, and where you indicate which body systems and issues you will be addressing.
The overview can be written in two paragraphs;
 In the first paragraph you identify the interrelationship between the symptoms and their causes. In other words, connect the main concern(s) to the imbalances found in the LAF, NSP and NSP sub questionnaires. Also, identify any interconnections between body systems if relevant. This supports the basis for your recommendations.
 Always reference the relevant body systems or subsystems from your NSP summary to the participant’s main complaint. For example, if a participant is concerned with constipation and she has numerous scores for an underactive stomach in the digestive system, then you will discuss her constipation in relation to the effects of an underactive stomach on the colon.
 In the second paragraph, write a short summary of your plan of action or how you will address the main concern(s) and their underlying causes. (Create a plan of which body systems you will address in the first report and which body systems you will address in the follow up appointment).
 Specific Recommendations for Client (1,200-1,300 words):
a) Diet: What changes to the diet would you recommend? Provide specific food
examples for each meal of the day and provide your rationale. (700-800 words) Note: The 3-day menu requirement indicated below does not replace the details needed in your dietary recommendations.

In total, your complete assessment should not exceed 5 pages including your menu plan page. Be sure to clearly print your name on the cover page. Use titles and subtitles throughout the report to identify each separate section and avoid terminology such as “treat”, “cure”, and “heal” as these words are outside of your scope of practice and CSNN discourages their use in your case studies.
