Self Evaluation Form
The Self-Evaluation Form is due in this Module. An electronic copy of the self-evaluation form may be downloaded from Doc Sharing.

Please analyze the eleven principal competencies and seven principal standards and place a check (by hand or electronically) in each of the appropriate boxes. You may consult with your Site Supervisor, yet this is your self-evaluation. Write the initial dates of contact with your Field Supervisor and any comments you would like to add.

The purpose of this Texas Education Agency (TEA) required form is to document the initial contacts made between your Field Supervisor and you. The documentation of the contact dates is required. After this form is completed then it should be uploaded to the appropriate Dropbox and then scanned to Folder #1 of your final ePortfolio.
