Translate the VOC into the VOP. Using the HCAHPS survey from CMS, develop a measurable indicator for the first 25 questionnaire items. Provide an example for each of the 25 items. Here are a couple of examples to help get you started:
1. How often did nurses treat you with respect? Percentage of times a nurse called a patient by his or her name.
2. How often did nurses listen carefully to you? Number of times per visit the nurse interrupted the patient.
