In looking at your materials, I can see that you:
you plan to conduct interviews, that will last approximately 30 minutes.Collect demographic information from participants, related to their age, gender, length of time working in or knowing of the hospital, their education level, and their ethnicity.For those who wish to participate, but cannot conduct an interview, they will be offered the option of completing a questionnaire; the questionnaire will include the above information, and can be completed (online? paper copy? please clarify).

If you are doing an online survey, you need to clarify that in your data collection instruments.
If you are referring to the demographic survey alone, you need to provide a link (no data can be collected, even demographics, until you have consented all individuals).
If you are referring to the option participants have to take the interview questions as a survey, and it will be online, then we need a full online activated survey, you need to put the link in the recruitment material, and you need to make sure that your online survey begins with our approved informed consent as the first page. At the end of the informed consent you must have a question about whether or not participants have read and agree to participate in your survey. This question must be required. If they click “agree” they will be allowed in to the survey. If they click “disagree” they should be exited from the survey, and not allowed to continue.

You need to revise your informed consent and make sure your information aligns across the application, and recruitment.

If your survey option is fully online, then you need a different informed consent for the online survey, and a separate one for the interview participants.
In the eligibility criteria, for example, you do not say anything about being a patient until after the exclusion criteria. Please clarify.
Under research activities, please clearly bullet research activities:
* answer demographic questions related to age, gender, length of time working at the hospital or knowing of the hospital
* clearly let participants know that you are offering an online survey or an interview (if they are doing an anonymous, online survey, they should already know that, but you need to state it).
In your application you state that you are doing both audiorecording and videorecording. Please add that to the interview informed consent.
