Write a Discussion on Alterations in Cellular Processes.

At its core, pathology is the study of disease. Diseases occur for many reasons. But some, such as cystic fibrosis and Parkinson’s Disease, occur because of alterations that prevent cells from functioning normally.

Understanding of signals and symptoms of alterations in cellular processes is a critical step in diagnosis and treatment of many diseases. For the Advanced Practice Registered Nurse (APRN), this understanding can also help educate patients and guide them through their treatment plans.

For this Discussion, you examine a case study and explain the disease that is suggested. You examine the symptoms reported and explain the cells that are involved and potential alterations and impacts.

To prepare:

Post an explanation of the disease highlighted in the scenario you were provided. Include the following in your explanation:

The role genetics plays in the disease.
Why the patient is presenting with the specific symptoms described.
The physiologic response to the stimulus presented in the scenario and why you think this response occurred.
The cells that are involved in this process.
How another characteristic (e.g., gender, genetics) would change your response.
Scenario 2: A mother brings her 6-month-old daughter to the HCP for evaluation of possible colic. The mother says the baby has had many episodes of crying after eating and, despite having a good appetite, is not gaining weight. The mother says the baby’s belly “gets all swollen sometimes.” The mother says the baby tastes “salty” when the mother kisses the baby. Further work up reveals a diagnosis of cystic fibrosis. The mother relates that her 23-month-old son has had multiple episodes of “chest congestion” and was hospitalized once for pneumonia. The mother wants to know what cystic fibrosis is and she also wants to know if she should have any more children.

As medical care evolves, so does the role and scope of practice of the advanced practice nurse in acute care settings. When providing patient care, however, you, as an advanced practice nurse, must always ensure that you are working within the scope of practice outlined by your state’s laws and institution’s regulations. For this Discussion, you examine these laws and regulations and consider how they might affect your future clinical practice, consider the credentialing process for your area, and discuss negotiation techniques you may use in a job search.

Locate a collaborative practice agreement. Based on this agreement, consider the scope of practice of an advanced practice nurse in your state.
Locate and select at least two hospitals where you would be interested in working at once you obtain your licensure.
Locate the hospitals’ credentialing documents and investigate the scope of practice at each hospital.
Note: All hospitals do not have the same scope of practice even if they are located within the same city and state.

Consider how the laws and regulations of the state and hospitals might affect your practice with medical and surgical patients in acute care settings.

Critical thinking is pivoted on elements supported in a five-step process. The steps include: the putting up of questions; research; inquisitions; review of the implications; and review of available options (Conway & McMillan, 2018). Asking of the initial questions helps with contextualization. It also points to the kind of information that should be integrated into the study. Research then helps with the gathering of information, the objective being to understand the problem and the related important theoretical frameworks. Inquisitions should then come with questions that explore the concepts, their relevance, related assumptions, and the sensibility of every idea that relates to the problem. In the review of implications, researchers should try to understand the effects of the decisions that they are bound to make. In the final step, they should consider any available alternatives, and how, if at all, they can improve the quality of the decisions arrived at in the first place.
The above processes are meant to support intellectual standards (which include clarity, accuracy, significance, fairness, breadth, sufficiency, logic, depth, precision) and intellectual traits (which include humility, autonomy, integrity, empathy, perseverance, and confidence) (Hyytinen, Toom, & Shavelson, 2019). The standards and traits contribute to good reasoning, but the guide to achieving them is based on the understanding that all processes of reasoning have a purpose, are triggered to solve problems, are originally based on assumptions, are executed based on points of view, are shaped by ideas and concepts, are characterized by inferences from which conclusions are drawn, and ultimately have implications/consequences. These points of consideration are also the elements of reasoning, and, together with the habitual practicing of the intellectual traits, can ultimately make it easier to critically interpret and apply information (Hyytinen, Toom, & Shavelson, 2019).
Given the time, I would like to execute all the stages outlined in the processes of critical thinking, and to do so in accordance with the above illustrated elements. Such a process would guarantee a more wholesome, accurate, and impartial development of the study. It would also encourage the involvement of the affected stakeholders, and it would help demonstrate that the ideals recommended in the study reliably address the problems that affect the industry. Take, for instance, that the consideration of alternatives to my conclusions would help show why my recommendations have more weight, a factor which in turn will help achieve positive implications for the study (Conway & McMillan, 2018). Putting up of questions from the beginning will also make it possible to ensure that all the successive steps of research are well-contextualized. Basically, each of the steps have an important contribution to make. While I do understand that facts and conditions do change, and that such changes may eventually prompt future researchers to either invalidate my conclusions or declare them as insufficient (if that would be the case), my objective is to do the best with the currently available facts and to ensure that my research processes are supported with valid processes of reasoning.

Healthcare organizations continually seek to optimize healthcare performance. For years, this approach was a three-pronged one known as the Triple Aim, with efforts focused on improved population health, enhanced patient experience, and lower healthcare costs.

More recently, this approach has evolved to a Quadruple Aim by including a focus on improving the work life of healthcare providers. Each of these measures are impacted by decisions made at the organizational level, and organizations have increasingly turned to EBP to inform and justify these decisions.

To Prepare:
